Hi Peter,
I think you should make Q. 3 either (a) conditionally required based on the response to the preceding question or (b) optional as I had to type in "Not Applicable" in the box in order to proceed. An easy way to fix this could be just to make it a multiple choice question (one selection permitted) with "not applicable" as the bottom option. 🙂

Thanks. I know the questions are a bit awkward and could be solved by making some of them conditional. I suppose that as much as possible I just wanted to allow people to type anything they wanted with some light prompts. But I need to upgrade the software (or embed an external survey) -- it was either get the survey out this year with the awkwardness or wait another year. So I hope we still get some useful stats and insights this year!

Oh yeah, embedding a Google Form would've been good (and easy) to do. Agree that making the questions conditional would be somewhat challenging on short notice and with the limitations in the software.
