Hello,
I am trying to transfer the TFSA money to newly created account in EQ Bank and need to submit the signed form. Failed to find one so I could sign.
When can it be downloaded?

The steps are outlined in their TFSA FAQ

... looks like you need to complete a soft form before it can be printed for signing.

EQs "live chat" is also quite good. I've just recently opened accts with EQ, and actually just mailed in our 'successor holder' documentation for the TFSAs, today.
Once I get confirmation, I'll start to move funds in.
